These Terms and Conditions apply to the promotional offers listed below. By accessing or using these offers, patients acknowledge and agree to the conditions set out here.
1. Eligibility
- All offers are available to new and existing patients of Dentalcare of Forrestfield, unless otherwise specified.
- Offers are non-transferable and limited to one use per patient.
2. Expiry and Availability
- Offers are ongoing unless otherwise specified or withdrawn by the practice.
- Offers are subject to appointment availability.
- The $599 Professional Teeth Bleaching offer is available on Wednesdays and Thursdays only.
3. Offers Included
a. Free* Invisalign Consultation
- Available to new and existing patients.
- Includes a consultation to discuss Invisalign treatment suitability.
- The consultation may involve a limited examination to discuss treatment options relevant to Invisalign.
- Diagnostic imaging, including X-rays, is excluded. Additional treatment or diagnostic procedures will be subject to standard practice fees.
- Health fund benefits cannot be claimed for this consultation.

b. Free* Dental Implants Consultation
- Available to new and existing patients.
- Includes a consultation to discuss dental implant treatment options.
- The consultation may involve a limited examination to discuss treatment options relevant to dental implants.
- Diagnostic imaging, including X-rays, is excluded. Additional treatment or diagnostic procedures will be subject to standard practice fees.
- Health fund benefits cannot be claimed for this consultation.

c. $599 Professional Teeth Bleaching
- Available to new and existing patients.
- Offer price: $599 for professional teeth bleaching treatment (standard practice fee: $1,200).
- Available on Wednesdays and Thursdays only.
- Includes professional teeth bleaching treatment only.
- Does not include professional cleaning. If a professional clean is required prior to treatment, this will be recommended by the treating practitioner and charged separately at standard practice fees.
- Health fund claims are applicable where eligible under the patient’s policy.
- Patients are encouraged to confirm their level of cover with their health fund prior to attending.
4. Terms of Use
- Offers cannot be used in conjunction with any other promotion, discount, or payment plan.
- Offers are non-redeemable for cash or any other service.
- The practice reserves the right to withdraw or modify offers at any time without prior notice.
5. Health Fund Clarification
- Where health fund claims are applicable, patients must hold an eligible health fund policy with sufficient benefits remaining.
- Dentalcare of Forrestfield is not responsible for out-of-pocket costs where fund limits are exceeded.
- Health fund benefits cannot be claimed for free consultation offers.
6. Scope of Offers
- Offers are limited to the services specifically described.
- They do not include follow-up appointments, additional treatments, or diagnostic services unless expressly stated.
7. Practitioner Details
All services are provided by registered dental practitioners at Dentalcare of Forrestfield.
Services under the $599 Professional Teeth Bleaching offer are provided by:
